Setelah kematian ibunya dan menghadapi gangguan ayah tirinya, Samantha/Sam (Julie Estelle) memutuskan menyewa kamar kost di pinggiran kota. Sam masih depresi karena kejadian di rumah serta mimpi-mimpi buruk yang terus menghantui. Hubungannya dengan kekasihnya, Agung (Evan Sanders), menjadi renggang karenanya.

Sam menyewa kamar kost di sebuah rumah yang tampak angker di seberang sebuah pohon beringin besar. Dari para penduduk, Sam mengetahui bahwa pohon itu diperkirakan sebagai pohon kuntilanak. Rumah kost Sam menyimpan misteri karena ada bagian yang dikunci rapat oleh pemiliknya.

Ibu Kost Sam, Yanti (Lita Soewardi), menceritakan mengenai sejarah bangunan yang kini menjadi tempat kost itu. Dahulu, sebuah keluarga produsen batik bernama Mangkoedjiwo membuat pabrik batik dan mess pekerja di sana, namun, terjadi kebakaran besar yang memusnahkan aset-aset Mangkoedjiwo. Hanya rumah itulah yang masih tersisa. Kini, cicit Panembahan Sakti Mangkoedjiwo, Raden Ayu Sukma Mangkoedjiwo (Alice Iskak) menyewakan rumah tersebut untuk kost. Perbincangan yang berubah ke mitos Kuntilanak, berakhir ketika Yanti menembangkan sebuah durmo yang digunakan untuk memanggil Kuntilanak. Entah kenapa tembang itu membuat Sam menjadi pusing. Tanpa sadar, Sam dapat menembangkan durmo pemanggil Kuntilanak dan mencelakakan orang-orang di sekitarnya.

Agung yang mencoba meneliti mengenai Kuntilanak dengan bantuan Iwan (Ibnu Jamil), mengetahui bahwa kuntilanak yang biasanya hidup di pohon, dipanggil oleh wangsit-nya dan keluar lewat media tertentu untuk masuk ke dunia kita.

Sam yang ikut bersama Agung ke rumah Iwan, membaca mengenai batik Mangkoedjiwo. Masyarakat sekitar percaya bahwa keluarga Mangkoedjiwo adalah penganut aliran sesat. Ia juga menemukan tulisan di sketsa batik: "sing kuat sing melihara" (yang kuat yang memelihara).

Sam berusaha menghancurkan media pengantar Kuntilanak di rumah kostnya. Tanpa sepenuhnya sadar, ia mulai pandai memanfaatkan kekuatan Kuntilanak untuk kepentingannya.

Film ini mengangkat kisah urban tentang tradisi keluarga kaya memelihara kekuatan jahat untuk menjaga kekayaan mereka. Sam adalah orang pertama di luar keluarga itu yang merasakan kehadiran makhluk halus peliharaan mereka. Cerita ini adalah paduan kisah hantu urban, sejarah keluarga Jawa yang kaya dengan kehidupan seorang gadis kota.
